The Diabetes Education Program is located on the lower level of the Woodstock Hospital in room L304. Enter the hospital using the Athlone Avenue entrance.

The Diabetes Education Program provides education and support to individuals and their families newly diagnosed or with established diabetes. Individual counseling and group seminars are offered. The diabetes educators will help you develop self-management skills such as blood glucose monitoring and meal planning. The Woodstock Hospital Diabetes Education Program is also an Assisted Devices Program (ADP) registered Insulin Pump Program.  

The following are a description of the programs provided:

Pre-Diabetes Seminar 

This is a two-hour seminar with a diabetes nurse and registered dietitian, which provides education regarding general lifestyle strategies and meal planning to help delay the onset of Type 2 diabetes. 

Individual Diabetes Assessment and Diabetes Meal Planner Education 

This is a two-hour appointment with diabetes educators that includes one-hour with the nurse for blood glucose monitoring and medications and one hour with the dietitian for diabetes meal planning and label reading.

Diabetes Seminar 

This is a half-day education program. The diabetes seminar will cover various diabetes topics that will include the following diabetes self-management skills from;

  • Self blood glucose monitoring 
  • Foot care
  • Health eating & mindful eating
  • Meal planning with basic or advanced carbohydrate counting 
  • label reading guidelines
  • Heart healthy eating
  • Active living
  • Medications & actions

Gestational Diabetes 

This is a two-hour appointment with diabetes educators that includes one-hour with a nurse and one hour with a dietitian to review the gestational diabetes meal plan and self blood glucose monitoring to help optimize blood glucose numbers throughout the pregnancy.

Individual Diabetes Visit for Insulin Starts and Adjustments 

This is a one-hour individual appointment visit with a diabetes nurse for the individual who requires advance education on their medication regimen or the start of insulin and/or adjustments of the insulin to help improve blood glucose control. Individual counseling and support for pattern management is also available for the person on an insulin pump (CSII).

Individual Nutrition Visit for Meal Planner Education 

This is a one-hour individual appointment with a dietitian for nutrition information regarding a diabetes meal planner, label reading and/or carbohydrate counting. The visit is tailored to the individuals needs and can include the following;

  • Meal planning with basic or advanced carbohydrate counting
  • Label reading guidelines
  • Sugars and sweeteners
  • Active living
  • Heart healthy eating
  • Weight control
