The Complex Continuing Care Unit is located on the second level of the hospital, Unit unit 2100.
Complex Care is a specialized, referral-based program that provides time-limited, restorative rehabilitative care to individuals with complex medical care needs. Complex Care is focused on an individual’s ability to stabilize, progress and maintain their activity tolerance and overall functional ability. With a highly skilled team of medical, nursing and allied health professionals, a coordinated plan of care will be developed based on individual and specific care needs. Complex Care provides low intensity therapy over a longer period of time to meet the individuals medical, physical and cognitive rehabilitative goals.
Complex Care provides transitional care to support individuals to return to a community-based setting (home, retirement home) or a hospital-based intensive rehabilitation program as appropriate.
