Important MEMO re: New MRI forms
As of February 1, 2018, the new MRI Requisition form is mandatory, and the Appropriateness Checklists is also mandatory in order to book an MRI of the knee or spine.
In the fall of 2016, the South West LHIN approved the Medical Imaging Integrated Care (MIIC) Project with a mandate to improve both patient access and appropriateness of medical imaging services in the LHIN, starting with MRI imaging.
Based on medical literature, including Choosing Wisely recommendations, radiological standardsandspecific surgical referral requirements, a new standard South West LHIN MRI referral form, and new Appropriateness Checklists for MRI studies of the knee and spine have been developed.
Electronic versions of the forms are available for download on the South West Primary Care Alliance website (www.swpca.ca). If you are using a different EMR system or are a paper-based office, the fillable PDF versions of the form are also available for download from the South West Primary Care Alliance website.

How do I access these services?
A written referral from an Authorized Healthcare Provider is required for all services except for the Ontario Breast Screening Program. Your Authorized Healthcare Provider’s office will make the appointment for you through Central Booking at WH and will provide you with the information for your scheduled exam(s) requested, clinical information and any preparations required for the exam. You must bring the requisition and your Health card to your appointment. Theexamwillnot be performed if there is no requisition for the exam.
Appointments for routine mammograms for women 50 and over can be made through the Ontario Breast Screening Program (OBSP) at 519 539-7838.
Where do I register for my first appointment and follow-up visits?
All patients for any Diagnostic Imaging exams must register in the Diagnostic Imaging Department reception.
Where is the Diagnostic Imaging Department?
The department is located on the main floor of WH just past the Ambulatory Care waiting area.
Who provides the services?
The Diagnostic Imaging Medical Radiation Technologists in Nuclear Medicine, X-Ray, MRI, CT, Mammography/OBSP and Bone Mineral Density are registered members of the College of Medical Radiation Technology of Ontario. ThediagnosticMedical Sonographers are registered with Sonography Canada. The College of Physicians and Surgeons of Ontario license the Radiologists.
