CT Spine Appropriateness Checklist

Hours of operation: 

Monday through to Friday, 8:00 a.m. to 5:00 p.m.  There are occasional evening bookings for some CT's that run until 8 p.m.

Exams include scans of the head, chest, abdomen, pelvis, extremities and vascular contrast studies.

C.T. stands for "computed tomography".  C. T. scans provide highly detailed images of every region of the body and are most commonly used to examine the brain, chest, abdomen/pelvis and back.  Scans of any other body region can also be obtained.  C. T. is useful to evaluate the body for signs of infection, masses, trauma, degenerative changes etc.

Your physician's office will make the appointment for you through Central Booking at WH and will give you a CT information sheet that documents the exam requested and any preparations required for the exam.  Also, bring a list of your current medications to your appointment with your Health Card. The radiologist will interpret the exam and the results will be sent to your physician within 3 to 5 days.

Please note, the CT Requisition Form and Spine Appropriateness Checklist are mandatory in order to book a CT scan of the spine. Electronic versions of the forms are available for download on the South West Primary Care Alliance website (www.swpca.ca). If you are using a different EMR system or are a paper-based office, there are PDF versions of the form that you can download and fill in from the South West Primary Care Alliance website.
